MAKE AN APPOINTMENT
APPOINTMENTS
To make an appointment, simply call our office at 201-763-6313 (Ext. #100) during normal business hours or use our 24/7, online
scheduling application located at the top, right corner of our website. New patients should arrive 15 minutes prior to their
scheduled appointment time to complete our office forms. For your convenience, these forms can be downloaded from our
website and completed prior to your appointment. Please remember to bring your current insurance card, a photo ID and your
copayment to each office visit.
walk-ins welcome
If you are a walk-in patient, our staff will make every effort to accommodate you within a reasonable amount of time. Please keep
in mind, unless you are having a true medical emergency, we are first obligated to treat our scheduled patients.
cancellations / no-shows
If you need to cancel your appointment, kindly give us at least 24 hours notice of your cancellation. This will provide enough time
to reallocate the appointment to another patient. Regrettably, we reserve the right to charge a $25 fee if you miss an appointment
or fail to provide adequate notification of your cancellation (<24 hours).
TEST RESULTS
You will receive prompt notification of any test result that requires immediate attention, otherwise, please allow 3-5 business days
to receive your results. Dr. Pappert (or a staff member) will call to discuss your test results and any necessary recommendations,
treatment or follow-up. If you do not hear from us within 5 business days of your testing, please call 201-763-6313 (Ext. #101) to
speak with our nursing department. Due to HIPAA laws, we are prohibited from faxing results and can only discuss your medical
testing with the individuals clearly listed on your Patient Authorization Form.
MEDICATION REFILL REQUESTS
via local pharmacy
Please call your pharmacy directly if you need to refill your medications between office visits. Your pharmacy will fax a Medication
Refill Request Form to our office, which will be completed and returned within 1-2 business days. This is the easiest and fastest
way to refill your medications, and assures the accuracy of the refill. You can also call our Medication Refill Request phone line at
201-763-6313 (Ext. 102) to request your refills. If your refill request is denied, please schedule an appointment at your earliest
convenience.
via mail-order pharmacy
Please mail, fax or hand-deliver a completed Patient Order Form to our office. We will either fax this completed form or mail the
written prescriptions to your pharmacy within 1-2 business days. It usually takes about 2 weeks to receive you medications via a
mail-order pharmacy, so please plan your refills accordingly. If your refill request is denied, please schedule an appointment at
your earliest convenience.

-- excessive use of pain medications / narcotics / sedatives
-- if you need an antibiotic, you need to be seen
PHONE CALLS / MESSAGES
business hours
If you require specific medical attention from our office during business hours, please call 201-763-6313 and the prompts will
direct you to the appropriate staff member and/or their office voice mail. For general office inquiries, dial 0 to be connected to our
front desk supervisor.
after-hours
If you require medical attention from our office at night or on weekends, please call our answering service at 201-743-9020. If you
leave a detailed message, Dr. Pappert will return your call within 15 minutes. Please keep in mind, we do not refill medications
when our office is closed.
EMERGENCY CARE
If you are having a perceived life-threating emergency (chest pain, shortness of breath, trauma), call 911 immediately or go to the
nearest emergency room. Do not drive yourself to the hospital. Upon arrival to the hospital, please inform the emergency room
staff that you are a patient of HUDSON COUNTY PRIMARY CARE and Dr. Jeffrey Pappert. After an initial evaluation, the emergency
room physician will contact Dr. Pappert to discuss any significant findings and your subsequent plan of care.
